After clicking the above page you will get a page, give tick mark on "I Have Read and Understood the Instructions."

Then the rest of the application will appear. After that put the various information of yours which is mentions at below:

1. Your Universal Account Number (UAN)

2. Your Valid Mobile Number

3. Your State

4. Your Office

5. Captcha Code

After that click on "GET PIN" button to get secret pin to your mobile number.

Click on "I Agree" field to confirm.

After that put Authorization PIN which you have got to your mobile number (You can get Authorization PIN by clicking on GET PIN button).

Then click on "Submit" button.
