PAN known as Permanent Account Number
1. Go to http://pancard.org.in select the "LOST / REPRINT / Second Original PAN CARD"
2. Click on "Apply Now" button , as you can see in the picture.
3. After that you will get a form page where you have to choose your category or which purpose you are apply the PAN card.
Those options are-
Individual (Personal)
Hindu undivided family (HUF)
Partnership Firm
Limited Liability Partnership (LLP)
Company
Association of Persons (AOP)
Trusts
Government
Body Of Individuals (BOI)
Local Authority
Artificial Juridical Persons (AJP)
For these i have chosen "Individual" for proceed.
4. In the PAN form there have six important section , you have to filled up those fields very carefully and all the information that you are given to the form must be correct.
Changes and correction in PAN data section

Date of birth section
Father's Name
Address section (Residence Address and Office Address)
Telephone and email id section
Income source section
Proof of address section
5. After writing all the fields correctly click on "Submit" button.
